Isolated fallopian tube torsion is a rare situation in reproductive-aged women. The gold standard for diagnosis is laparoscopic evaluation, and the treatment of choice is salpingectomy without oophorectomy to preserve fertility. Still, inpatient with a presentation of the acute abdomen or hemodynamically unstable, urgent laparotomy is the treatment of choice. Here, we reported a 15-year-old virgin girl presented with acute abdominal pain and evidence of adnexal torsion on the abdominal ultrasonography. Urgent laparotomy revealed an isolated right fallopian tube torsion. Due to irreversible necrosis of the tube, right salpingectomy was performed. It's crucial to consider isolated fallopian tube torsion as a potential cause of abdominal pain in reproductive-aged women and use appropriate diagnostic measures to diagnose it early and preserve their future fertility.

Background & Objective: Premature preterm rupture of membranes (PPROM) occurs in about 2-5% of singleton pregnancies and is known to cause one-third of preterm births. Our primary aim was to determine the maternal and neonatal outcomes in PPROM cases in mothers with a gestational age of less than 37 weeks.Methods: In this prospective cross-sectional study, eligible singleton women between 24+0-37+6 weeks of gestation with the PPROM enrolled who had referred to Ayatollah Rouhani Hospital in Babol, Iran, during the years 2019-2020. Maternal and neonatal outcomes were obtained by the checklist.Results: The mean age of the studied mothers was 29.3± 6.19 years, and their mean body mass index was 30.6 ±5. The incidence of chorioamnionitis at the gestational age of >32 weeks was more than that in women at gestational age equal to or over 32 weeks (P < /em>≤0.0001). Vaginal bleeding was almost more than twice as high in women with a gestational age of less than 32 weeks compared to those with a gestational age equal to or over 32 weeks (P < /em>≤0.0001). Neonatal morbidity was higher in all cases at less than 32 weeks of gestation (P < /em>≤0.0001). The neonatal mortality rate was 5.35%, but it was 25% at less than 32 weeks of gestation (P < /em>≤0.0001). The latency period greater than 7 days had more odds ratio for neonatal morbidity.Conclusion: Due to the high incidence of maternal and neonatal outcomes in gestational age less than 32 weeks, it is suggested that appropriate instructional materials and proper proceeding should be taken to prevent preterm labor and preterm rupture of the membranes.

Mature cystic teratomas, also named dermoid cysts, are the most benign ovarian germ cell tumors and can occur in any age group. Transformation of the mature elements within the dermoid cyst to the malignancy termed mature cystic teratomas (MCTs) with malignant transformation occurs in only 1-2 % of cases. Of these, squamous cell carcinoma (SCC) with 80% prevalence is the most common type. A 54-year-old peri-menopausal woman -gravida 5, para 4, live 4, and abortion 1- presented to our clinic with a complaint of abdominal pain for about five months. Transvaginal ultrasonography revealed a mass with a diameter of 125-millimeter in the left adnexa with a complex appearance containing fine-echo liquid, solid internal component, and thickened wall. Spiral CT scan of abdomen & pelvis showed 147×121-millimeter bilocular lesion consistent with an ovarian dermoid cyst. Left salpingo-oophorectomy was performed, and a frozen section was requested during the surgery, which showed a malignant epithelial tumor consistent with squamous cell carcinoma arising in teratoma. Total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic lymphadenectomy, and partial omentectomy were done successfully. The patient was discharged from the hospital after two days without any complaints or adverse events. Regarding the rarity of MCT with malignant transformation, it is recommended to consider patient characteristics, preoperative imaging, laboratory evaluations, and the results of the frozen section during surgery since they can significantly change the treatment plan and the prognosis of the patient.
